生物可降解關節植入物及其製備方法

類風濕性關節炎會造成患者疼痛腫脹與關節的活動度下降,目前在手指的關節炎治療上,小關節置換術是手外科手術中的挑戰,已經為重建手術開發了幾種治療方式,包括切除和軟組織介入置換術,及矽膠人工指關節置換術。Swanson在1960年代提出的矽膠人工指關節置換術,是目前治療類風濕性關節炎的掌指關節破壞(MCP)關節最常用之方法。然而,在長期超過10年術後追蹤中發現,矽膠人工指關節的斷裂和磨損、以及人工指關節周圍的骨破壞,會隨時間的增加而惡化。本發明希望利用可降解高分子如聚己內酯(PCL)製成的生物可吸收式人工指關節當作臨時的支撐架,並把外型設計得能良好固定在關節處,使其活動時不會造成支架鬆脫滑動。指關節上包覆含有藥物及生長因子的奈米纖維薄膜誘導人體組織生長,藥物使用了萬古黴素與頭孢他錠兩種抗生素以及利多卡因當作止痛效果,並加入了結締生長因子促進結締組織生長,在可降解指關節逐漸降解並被人體組織替代後,可以在指關節處形成結締組織保護關節,同時形成人體新的指關節。 Totally degradable finger joints fabricated using 3D printing or injection molding in combined with electrospun drugs and biomolecules-eluting nanofibers have been developed. The biodegradable joint is a single piece flexible joint replacement implant for the metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joints in the hand; commonly due to rheumatoid or osteoarthritis. When implanted at MCP or PIP joints, the degradable joints allow for correction of deformity, and improvement in the function and appearance of the hand, and provide sustained release of antimicrobial agents for infection control and analgesics for pain relief. The joints degrade over time after connective tissues have grown at the joint regions, and bones have been regrown at the orthopedic sides. The joints also offer controlled release of biomolecules such as connective tissue growth factors and bone morphogenic protein to enhance healings for treatment of finger joint disorders. Methods of making these joints via heat processing and the making of nanofibers via electrospinning, as well as their uses in medical applications, are also provided.
