技術說明

類核苷酸化物已被廣泛用於抗病毒及抗癌藥劑。然而臨床實驗發現抗病毒治療在長期服用單方劑型後有相當比例患者會出現抗藥性病毒株，而使血清中病毒活性再度提升並使病情再度加重且須重換新藥劑。而在抗癌方面，類核苷酸化物亦遭遇到抗藥性癌殖株產生的嚴重問題導致化療失效˙更甚者，長期使用某些類核苷酸藥劑能干擾粒線體或核基因複製與轉錄而使一些長期服用之患者可能產生程度不等之副作用，例如類粒線體疾病症狀等。而類核苷酸化物抗癌藥劑更能造成骨髓抑制(bone marrow supression) 等不等程度的副作用與併發症。同時也限制了最高用藥劑量而更容易誘導癌細胞抗藥性的產生，進而造成臨床失敗。近年來，各項研究發現以複方之雞尾酒療法處理較單方更能改善抗病毒及抗癌治療之臨床預後。然而傳統之雞尾酒療法仍侷限在巨觀的投予口服或靜脈(或肌肉)注射特定比例之不同藥劑。而無法確認在需要被治療的摽定細胞仍有均等的藥劑比例及總量吸收。 Nucleosides and other compounds that mimic nucleic acid have been developed and widely applied to anti-viral and anti-cancer therapies. However, reistant viral strand and cancers may develop after treatment for certain period of time and led to clinical failure. Besides, as these compounds may also interfere with the metabolic pathway of nucleotides, involved in the DNA synthesis and/or transcriptional activity as well as mitochondrial DNA replications, side effects and clinical complications such as mitochondrial disease like syndrome may become important issues. Cocktail regimens have been developed and evaluated in clinical trials. However, batched delivery of the cocktail regimen to destinated infected or cancer cells based on nanometer sized biocompatible nanoparticles, which enabled better control of the delivery and activation of the drugs, have not been reported before.
